3D Nipple and Areola Tattooing After Mastectomy: Finding the Right Artist

This is usually the last appointment of a very long year, and it should feel different from all the ones before it. Choosing who does it is worth taking your time over — the technique is specialised, the results vary widely between artists, and there is no rush at this stage.

The timing question

Most people are ready once reconstruction is finished and the tissue has settled, commonly three to six months after the final surgery, and always with your surgeon's agreement. Skin that has had radiation needs longer and a gentler plan. If you are still having revisions, wait — tattooing into tissue that is going to change shape means tattooing something that will move. What separates good work from average work

• Dimension, not a circle. A flat disc of colour reads as a sticker. Real depth comes from highlight and shadow built to suit the light your body is actually seen in.
• Colour inside your own range. Areola colour is a muted, complex version of your skin tone, not pink. Pigment that heals too warm or too cool is the most common giveaway.
• Honest asymmetry. On a unilateral match, the goal is your other side — including its irregularities. Two identical circles look manufactured.
• Working around scars. Incision lines run through the placement area constantly. Scar tissue takes pigment differently and has to be planned for, not ignored.

Questions to ask before you book

• May I see healed photos — six weeks out — of unilateral and bilateral work?
• Have you worked on radiated skin, and how did you adjust?
• How do you map placement, and do I get a say before any needle touches me?
• What happens at the second session, and is it included?
• How will this look in five years, and what does a refresh involve?

Comfort is part of the technique

There are not many artists doing this at a high level, so most people travel, and two visits is the usual commitment.
